Your Ministry Reference Form is VERY important for several reasons:

• -It tells us that you are involved with your church through attendance and/or service...
• -It gives your referrer an opportunity to share their knowledge of you with us...
• -It helps us determine those who we feel are being led by the Spirit of God, have a heart for ministry to teens (and not just a fun week with a paycheck), and will fit in with our philosophy of ministry.

***PLEASE NOTE***
This must be filled out by a person in ministry who can speak knowledgeably about your spiritual life over the past year. If your pastor from home has not seen you enough to adequately comment, please find someone you have had regular contact with over the course of the year. This is not necessary limited to pastors and may include: youth pastors, mentors, ministry partners (one who is in an oversight position over you), or college advisors/professors (if attending a Christian college). Please do not seek out peers to complete your reference. Again, please try to find the person who is best suited to give an evaluation of yourself over the past year.
